    Includes:

    • Curated educational resources for families and caregivers

    • One (1) hour of in-person coaching

    • Guidance tailored to your immediate questions and concerns

    This option is ideal if you are early in the journey or need professional reassurance and direction without a long-term commitment.

  • $2,000.00

    Our most popular option for families seeking structure, clarity, and ongoing support.

    This level combines education, practical planning, and hands-on coaching to help you move from feeling overwhelmed to feeling prepared and supported.

    Includes:

    • Comprehensive educational resources

    • A standard dementia care plan

    • Five (5) hours of in-person coaching

    • Support implementing strategies that fit your loved one and your family

    This option is best for families who want a clear plan and expert support while navigating day-to-day care.

  • $4,500.00

    Intensive, high-touch support for families facing complex needs, rapid changes, or a season of transition.

    This package offers personalized guidance and consistent in-person support over a 30-day period, ensuring you are not navigating this alone.

    Includes:

    • Comprehensive educational resources

    • A fully personalized, professionally printed dementia care plan

    • Twenty (20) hours of in-person coaching within a 30-day period

    • Ongoing adjustments and support as needs evolve

    This option is ideal for families experiencing increased care demands, transitions in living situation, or heightened stress who need hands-on, consistent professional support.

  • Additional In-Person Coaching
    $200 per hour
    Available for continued guidance, problem-solving, and hands-on support beyond your selected package.

    Care Plan Updates & Revisions
    $300 per update
    Includes review of the existing care plan, thoughtful revisions based on evolving needs, and delivery of an updated written care plan.

    Please note: Care plan updates that require in-person coaching or extended support may incur additional coaching hours at the standard hourly rate.
